Instructions
Select the > symbol to open the form that has been Accepted by the Community of Faith
2. Review the form completely. Once you have reviewed the form, select this button to View Standard Terms.
3. Review the terms and select I Understand. This will return you to the form.
4. Select BOTH “I agree” buttons and then choose Accept or Decline (if you choose Decline you must enter your reason for decline, and the form becomes a draft for the Community of Faith to make changes/edits).
